Community Activities

Disability Support

Registered Nurses

We help you develop daily living skills as well as support you with community access.

For people requiring specialised Disability nursing care for more complex needs. We have RNs that specialised in:

  • Enteral feeding (PEG/JJ/NGT).
  • SPC/IDC management.
  • All routes medication administration.
  • Diabetes management.

Support Coordination

Accommodation & Housing

Respite Care

At Outreach Care, we take the time to get to know you and understand your goals so we can offer you quality support coordination services (Level 1,2 and 3).

  • Assist you to get the supports in your plan started
  • Work with you to find mainstream services for you to access such as employment and education
  • Ensure that you are linked in to services you want and/or need
  • Work with you to develop skills and resilience
  • Parenting training
  • Respond to crisis
  • Build your skills and increase your capacity
  • Work with you to achieve outcomes

We offer assistance with daily like tasks in Supported Independence Living (SIL) or short term accommodation.

This means you need a significant amount of help throughout the day, 7 days a week. This includes overnight support.

SIL is best suited to people with a disability who have higher support needs.

To the family members who are the primary caregivers for an NDIS participant, there can be times when they need some respite. Respite care allows both a person with a disability and their carer/s to have a break or time off. It is the alternative care that is put in place while a usual care giver is absent.
